NuMed Urbana available for patient registration / transfers

NuMed’s Urbana location has been officially licensed and approved to open by the state of Illinois, and now appears on the Illinois Department of Financial and Professional Regulation’s list of licensed medical cannabis dispensaries.

Patients must select a dispensary with the Illinois Department of Public Health; visit the DPH’s site for more information on selecting or changing your medical cannabis dispensary.

• Patients who have received their medical cannabis card should complete and return this form to the DPH.
