Set up an appointment with a doctor, physician assistant, or nurse practitioner to confirm your qualifying condition and process your qualification registration.
Identification – make sure you have a valid non-expired IL Driver’s License or State ID plus two 2×2 inch passport pictures for the registration application process.
Get yourself online and register through the state’s online application portal.
